The Challenge

I was asked to add an e-form feature that allows providers from different clinics or independent make custom forms based on their client's needs. The challenge was to make it flexible, so providers could change the forms as much as they needed while collecting client information efficiently.

Task:

My task was to build a platform where providers could easily create customized digital forms, edit them, review them, and also create and send contracts and intake forms to their clients.

Process:

I worked along with the marketing team by conducting multiple interviews with different providers like doulas, midwifes, childbirth educator and so on, to understand their needs and necessities.  This ensured that their ideas influenced the design and how it worked.

What providers said during the interview?

  • They needed pre-made forms, as well as the ability to create new form freely from scratch
  • Be able to create intake forms (a document used to gather important information from clients, customers, or patients when they first interact with a service or organization*) and contact forms
  • The ability to archive old forms or modify them
  • Be able to know if their client has or hasn't completed the form and the ability to resend it.

Wireframing

To meet the two-week timeframe for implementing this new feature, I initiated the creation of high-fidelity wireframes that could be implemented and tested with our providers. I created and completed a simple UX/UI workflow where providers could make, edit, and review forms easily. We had regular feedback sessions, letting providers test and improve their forms. Along with the developer we added a secure data system to gather and keep client info safe following the HIPAA Compliance requirements.

Template page - Providers have the ability to drag and drop any item form the lef panel and customized it.

On the left-hand panel, providers can hide the field and make it visible only to themselves or designate the field as required, all while changing the label name to suit their preferences.

Providers are able to archive or modify their forms

When providers navigate to the Clients' information and click on eForms, they will have the ability to check the status of the form. If necessary, they can also resend it to their client or attach a new form.

Results:

Thanks to the improvement Treehouse was able to increase their membership subscription (between 5 to 6 members each week). We also allowed providers to get a 7 days trail.  In the past providers were using a third party company to create e-forms and were only using the free version of Treehouse. However with the new feature implemented providers felt that they have more flexibility and more ways to manage their clients and services.
